New 2026 LER Codes
SVS is aware of denials for payment related to the new LER codes. On February 5, Noridian, the local Medicare Carrier for Alaska, Arizona, Idaho, Montana, North Dakota, Oregon, South Dakota, Utah, Washington and Wyoming stated “Effective January 1, 2026, CPT code changes for certain endovascular procedures resulted in an inadvertent claims processing error. This error caused some claims for services performed in office-based settings to deny incorrectly. Claims will be reprocessed.” Noridian went on to say “No provider action is needed to correct the claims. Providers should not resubmit claims unless otherwise instructed.” Read the full Nordian statement.
SVS members experiencing similar denials from other Medicare carriers should forward redacted denials to the SVS via the Coding Topics Submissions form.
