Vascular Care for the Underserved Project Grants

The SVS Foundation's Vascular Care for the Underserved Project grants support initiatives explicitly designed to explore solutions to the significant disparities in vascular surgical care delivery across the United States.

Up to $150,000 will be awarded to SVS members for an innovative, community-based initiative. Priority is given to projects that provide outreach, screening, and other resources to patients impacted by lack of access, inadequate resources, or distance from modern healthcare facilities.

Successful pilot projects will combine strengthening local resources for patient care with expert referral networks for patients in need of advanced or immediate treatment.

  • The pilot projects' focus is on defining and addressing barriers to care and providing education to both local community healthcare providers and their patients.
  • The intent of the pilot projects is to increase awareness, identify and prevent vascular disease, improve pathways to treatment, and optimize the use of existing local providers and referral pathways.  Pilot project support is not intended for direct referrals to specific providers or specialists.

Ideal proposals will describe a multidisciplinary approach to address some or all of the following goals:

  • Clearly define the underserved community your project is targeted to benefit
  • Define the perceived need regarding vascular health care access and vascular disease outcomes
  • Directly identify and assist local providers and other service organizations with addressing significant gaps in knowledge, clinical examination methods, point of care diagnostics, and other determinants of vascular health and disease
  • Directly identify and empower patients with improved knowledge about risk factors and early signs of vascular disease and the available vascular care resources in their community
  • Develop solutions to address the barriers to vascular health care that are actionable, scalable, and sustainable

Funding

Over two to three years, up to $150,000 will be awarded for direct costs. The grant does not support office construction or clinic enhancement projects, practice marketing initiatives, physician salary or honoraria, overhead expenses, or major capital budget items.

The SVS Foundation does not support indirect costs.

Eligibility

The Principal Investigator may submit only one application for the Vascular Care for the Underserved grant. The Applicant must be an SVS Active Member. Candidate members, trainees, fellows, and residents are not eligible.

Application Process (application deadline - to be announced)

1. Application Form Includes: 

  • Principal Investigator (PI) name and contact information
  • PI Institution
  • Project title

2. Principal Investigator Curriculum Vitae and NIH biosketch

3. Project Proposal - Not to exceed 8 pages total

  • 1 page - Principal Investigator statement outlining his/her practice setting and relationship to the community being studied, including delineation of prior experience with community-based projects.
     
  • 3-5 pages - Project description including:
    • Problem being addressed/community to be served/specific aims
    • Statement of innovation and impact
    • Project timeline
    • Resources needed
    • Available expertise and resources
    • Connection with community healthcare providers
    • Methods to measure project success and attainment of stated goals
    • Future plans/sustainability
  • 1-2 pages - Budget and budget justification
    • Itemized expenses
    • Budget justification
    • Percent FTE for a project for PI(s) and staff
    • List of any other funds already secured or applied for

4. Additional documents

  • Letter(s) of support from institutional leadership, collaborators, and community partners

Reporting Requirements

Grant recipients will be expected to provide brief 6-month reports. Continuation of annual funding will be dependent on successful progress. At the completion of the project, the awardee must submit a report to the SVS Foundation. The report criteria will be provided to the awardee at a later date.
